Submission criteria

Please note that only U.S. residents can participate in Who's Your LifeLine? program.

You'll need the following items for the submission form:

• A short essay (between 250-750 words) answering the question, "Who is your LifeLine, and why?" You can enter your essay directly into the form, or you can upload your essay in a Word document. Your essay must follow the submission guidelines below.
• Permission from your nominee. The form asks you to certify that the person you nominate approves the nomination and is aware and approves their information being shared.
• Contact information for yourself and your nominee, including name, phone number, email address, and date of birth.
• (Optional) A photo of your nominee, JPEG format, size 100KB or smaller.

Program Guidelines

Your submission for the Who's Your LifeLine? program should abide by the following guidelines:

• Submission cannot mention any specific medical treatment for MS (branded, generic or alternative therapies)
• Nominators must verify that their nominee is aware and approves of their information being shared. A signed consent form will be required if your nominee is selected as a LifeLine Spotlight
• Submission cannot mention adverse events due to medical treatment
• Submission must focus on how the nominee meets each of the four scoring categories:
  • 1. Attitude/Determination: What is the nominee's attitude toward MS and how has he/she taken steps to overcome being affected by the disease (either as someone living with MS or as a caregiver/supporter)?
  • 2. Role Model/Inspiration: Does the nominee act as a role model to others who are affected by MS?
  • 3. Community Service/Involvement: Has the nominee established himself/herself as an active member in the MS community?
  • 4. Achievement: How has the nominee worked toward achieving his or her personal goals or helped the person with MS whom they care for to do so?
• Submission must be within the word limit (minimum of 250 words, maximum of 750 words)

Each submission will be reviewed by a panel of three members of the MS community, and scored based on the program guidelines. From these submissions, the panel will periodically choose nominees for the LifeLine Spotlight. These inspirational individuals will be recognized on mslifelines.com, so that the MS community can benefit from your LifeLine's unique story. Also, from now through June 24^th, 2010, all nominees will be eligible to be featured in an upcoming issue of REDBOOK magazine if they are selected as a LifeLine Spotlight.

Please note that by nominating your LifeLine, you are granting permission to a Who's Your LifeLine? program representative to contact you in the future regarding the status of your nomination. If your nominee is selected as a LifeLine Spotlight, a representative from Who's Your LifeLine? will contact you as well as your nominee to obtain a signed consent form and physician's signature, if applicable, before their selection can be considered final.

Nominations will be accepted beginning 4/13/2010 at 12:01 AM (ET) until 6/24/2010 at 11:59 PM. Open to legal residents of the 50 United States and the District of Columbia and Canada who have reached the age of majority in their state or province. Void in Puerto Rico, Quebec Province and where prohibited by law.
